3. I am an existing patient AND I have been seen at Women’s Healthcare of Illinois since Jan. 2009, AND I have never created a Patient Portal Account.  If you can answer yes to all options, you can create your patient portal account but you do not have to complete either the health history or registration forms unless you need to make changes since we last saw you.
